What causes a drive belt tension pulley to break?

I was going down the highway and my power steering lock up and temp gauge went into the red. It was defiantly my drive belt but when i looked at it my belt is in prefect condition. the belt looked like it came off the tension pulley and the pulley wheel broke down the middle. what could have caused this?

I had a tensioner bearing seize (I assume) and snap the tensioner off of the front of the block. Belt was in great shape, no other accessories or idler puller was bound.

these types of things happen. I wouldn’t worry about it unless you start breaking a bunch of them.
